If you NEW to CDPAP
Check Eligibility
Determine if you're a Medicaid recipient and resident of New York State who needs daily living assistance.
Health Check
Visit your primary doctor’s office to document your needs.
Caregiver Selection
Select a trusted individual as your caregiver, excluding spouses and designated reps.
Care Provided, Caregiver Rewarded
Once approved, enjoy ongoing support and caregiver get payments for services through the program.
