IF YOU USE MOBILE HEALTH 


🚨 PLEASE FOLLOW THE INSTRUCTIONS VERY CAREFULLY TO ENSURE YOUR PA SERVICE WITH PPL DOES NOT STOP WORKING ON OCT 1st. READ EVERYTHING CAREFULLY:

🚨If your MobileHealth Appointment is after Sept 17, we recommend scheduling with your PCP to get an earlier time as appointments after Sept 17 with MobileHealth do not ensure that you will be allowed to work October 1st. Please try other PCP’s if you cannot get an earlier time (Check with your insurance). 


If you schedule an appointment with MobileHealth, please ensure ALL steps are completed correctly: 


  •  This form will be completed for you by MobileHealth at your appointment : 


  • Make sure your immunizations and TB Risk Assessment is Complete.  Confirm this with MobileHealth.



🚨 INSTRUCTIONS ON UPLOADING…

1. To Upload to PPLFirst, Log in to https://account.pplfirst.com/s4s/ to your PA account to upload physical, immunizations, drug assessment form and any scans/X-Rays to the Required Documents tab.



Check on your status in several days/week with MobileHealth or your CDPAP, Quality Touch.

IF YOU USE YOUR PERSONAL PCP/DOCTOR

If you schedule an appointment with your doctor, please ensure ALL steps are completed correctly: 

*ALL FORMS MUST BE UPLOADED TO MOBILE HEALTH TO BECOME EFFECTIVE.

1. To Upload to PPLFirst, Log in to https://account.pplfirst.com/s4s/ to your PA account to upload physical, immunizations, drug assessment form and any scans/X-Rays to the Required Documents tab.



🚨YOUR DOCUMENTS WILL NOT BE PROCESSED BY PPL IF THEY ARE NOT UPLOADED TO MOBILE HEALTH.

TIME EXEMPTION CASES and Rubella rubeola, TB, and PPD

(New PA’s Must Complete all Four)



Drug Test and PPD, TB/quantiferon must be completed annually. 



COMPLETED X-RAYS

Chest X-Rays will be effective for 10 years after your initial X-Ray screening was received. Please confirm the result date and upload to Mobile Health and PPLFirst Required Documents.  

WITHOUT X RAY

If a PA did their Quantiferon/PPD or TB, and does not have an X-Ray result, and they are negative, please upload the negative results to MobileHealth and PPLFirst Account (Instructions Below) 

*If it is close to your expiration date, it must be re-updated.


Rubella rubeola (MMR) 

PA’s  must show proof of immunity. If previous screenings show immunity, they can be reuploaded to MobileHealth and PPLFirst (Required Documents- Under PA Physical) (Link)

If PA is not immune, your physical will not be cleared/processed. Please contact your PCP for a consultation to review your options.
